1) Please click on the preferences icon as shown below. Navigate to medical record then history/exam forms and select the relevant option.
2) Click on Add History Form / Add Exam Form.
3) Fill in the necessary details and click on submit.
4) Once you see the name you have just saved. Click on manage form.
3) Click on add components to add a new question or statement.
4) Add the information according to the image shown below.
i) add the Question or the header.
II) select the type of options you get to choose the answers from.
III) Here with the first option you can setup a report card if necessary and with the second option you can add an attachment.
IV) Here you can add in all the options you would want to display while attending the patient.
5) Follow the same steps to add more questions and options. You can also mark a question mandatory if required by checking the box next to the question.
