These settings can be setup once you have subscribed to SMS feature.
By default all client mobile numbers are flagged not to receive SMS. We expect that clinics will obtain authorization to send SMS to a particular client. During the appointment process there will be an option to turn on SMS for each client. Clinics will also be able to flag it on at the time of adding a client and editing the client information. Mobile numbers can be added at these stages as well.
